Sulphur Springs Service Coordination

The Sulphur Springs IDD Authority Service Coordination office assists individuals with intellectual and developmental disabilities. Service Coordinators help individuals access medical, social, educational, and other services to improve their quality of life and their level of community participation.

Sulphur Springs MH Clinic & Substance Use Disorder Services

The Evaluation Center comprises a team of licensed professionals that will meet with individuals to determine their needs and strengths based on various assessments. These assessments give the best picture of clinical needs so that we may refer the individual to services tailored to achieve their recovery goals. The Evaluation Center staff are here to partner with individuals to develop their recovery plans. The staff will link you to the Resource Center or provide you with community referrals if needed.

The Resource Center is a comprehensive, out-come oriented rehabilitation program designed to meet the individualized needs of program participants. The staff in the Resource Center gets its “directions/orders/prescription” from the Evaluation Center in a Person-Centered planning format that outlines goals, objectives, and interventions.

It also provides outpatient behavioral health services for individuals and families who are in crisis or who need help with ongoing problems caused by mental illness, intellectual/developmental delays, or substance abuse.

Early Childhood Intervention (ECI) helps babies and toddlers, ages birth to 36 months, with disabilities or delays in their development. ECI supports families through education and family services to help their children reach their potential. Families and staff work as a team to develop a plan outlining services for the child and family. Most services are provided in the home or community to help children learn the basic skills that are a part of their family’s daily routines. Services are provided in the home and community settings and help children learn basic skills as part of their family’s daily routines.

Home & Community-based (HCS) Services

Lakes Regional Community Center provides HCS services to Texans with
Intellectual Developmental Disabilities and/or related conditions. Our Person-centered Planning approach provides HCS services and supports that meet the needs of each individual. We engage individuals and their families throughout the planning phase and delivery of HCS services, from deciding on program oversight to choosing their living arrangements, therapies, attendants, daily activities, and more.

The Lakes Regional team works with the individual and their family to create a comprehensive plan through which services are incorporated into the individual’s day-to-day life. The plan is flexible and is revised as often as necessary to meet the changes of the individuals. The Lakes Regional Provider team strives to ensure that every individual we serve is completely satisfied with all services we provide.

Services include Host Home/Companion Care, Group Home; Supervised
Living or Residential Support Services, Community First Choice (CFC),
Nursing, Behavioral Support, Adaptive Aids, Minor Home Modifications,
Specialized Therapies, Dental Treatment, Day Habilitation, Employment
Assistance, Supported Employment, and Respite.
