Sulphur Springs Early Childhood Intervention Office

Early Childhood Intervention (ECI) helps babies and toddlers, ages birth to 36 months, with disabilities or delays in their development. ECI supports families through education and family services to help their children reach their potential. Families and staff work as a team to develop a plan outlining services for the child and family. Most services are provided in the home or community to help children learn the basic skills that are a part of their family’s daily routines. Services are provided in the home and community settings and help children learn basic skills as part of their family’s daily routines.

Rockwall Connections Training Center

The Rockwall Connections Training Center was established to provide day habilitation services for individuals ages 18 and older. This program consists of long-term personal and social development opportunities within a structured environment for individuals with developmental disabilities. Services may include daily living skills instruction, basic education, recreational and social activities, and other forms of developmental support that help participants establish and maintain the functional skills required for community involvement, self-advocacy, self-care, and completive employment.

Intermediate Care Facilities (ICF) Services

The Intermediate Care Facilities for Individuals with an Intellectual Disability (ICF/IDD) program is long-term care for adults with intellectual and developmental disabilities. The ICF/IDD Program has residential facilities for people needing treatment in a supervised 24-hour setting. The state manages these facilities, which normally have 6 residents, have three to four bedrooms; therefore, the individuals share rooms, and the houses are similar to homes in the area. The cost of living in an ICF/IDD facility is covered as a Medicaid benefit. Lakes Regional provides residential services to individuals according to availability.

Each individual’s program is coordinated and monitored by a Qualified Intellectual Disability Professional (QIDP) is done on a person-centered planning approach. Additionally, each individual receives continuous active treatment in independent living skills and personal care skills from highly trained staff.

Other services provided are Nursing Services, Medical Care, Psychiatry Services, Dental, Adaptive Aids, Behavior Support, Day Programming, Transportation, Community Activities, and more intense support services to help each individual remain in the community setting.

Each person’s program plan is developed according to that person’s needs and is reviewed and updated as needed to capture the changes that occur in an individual’s life.

Home & Community-based (HCS) Services

Lakes Regional Community Center provides HCS services to Texans with
Intellectual Developmental Disabilities and/or related conditions. Our Person-centered Planning approach provides HCS services and supports that meet the needs of each individual. We engage individuals and their families throughout the planning phase and delivery of HCS services, from deciding on program oversight to choosing their living arrangements, therapies, attendants, daily activities, and more.

The Lakes Regional team works with the individual and their family to create a comprehensive plan through which services are incorporated into the individual’s day-to-day life. The plan is flexible and is revised as often as necessary to meet the changes of the individuals. The Lakes Regional Provider team strives to ensure that every individual we serve is completely satisfied with all services we provide.

Services include Host Home/Companion Care, Group Home; Supervised
Living or Residential Support Services, Community First Choice (CFC),
Nursing, Behavioral Support, Adaptive Aids, Minor Home Modifications,
Specialized Therapies, Dental Treatment, Day Habilitation, Employment
Assistance, Supported Employment, and Respite.

Texas Home Living (TxHmL) Services

Lakes Regional Community Center provides TxHmL Services to Texans with Intellectual and Developmental Disabilities and/or related conditions. Outpatient Biopsychosocial IDD Services treats individuals with co-existing intellectual and developmental disabilities and mental illness. Individuals may also present with behavioral challenges influenced by their co-existing diagnoses. Based on a biopsychosocial approach, our specialized team of clinical professionals serves individuals by providing psychiatric services, counseling, clinical assessments, treatment planning, advocacy, and various other interventions.

Outpatient Biopsychosocial Intervention is a program that provides services tailored to each individual’s needs and preferences based on the principles of empowerment, choice, building from strengths, and honoring dignity.

Greenville Training Center

The Creative Expressions Art Studio provides opportunities for individuals to participate in visual and performing arts as of the Training Center.
